What to Bring...
to the First College Planning Meeting
Please bring with you as many of the following items as are
appropriate for you:
- Current report card and year-end report cards for the
past three years
- Teacher progress reports or comments
- Standardized testing results (SSAT, ISEE, PSAT, SAT, ACT,
etc.)
- ON CD or USB DRIVE: List of most important school-related
activities, outside activities, religious activities,
hobbies and interests, community service, summer experiences,
and honors. Organize the list by activity, from most important
to least important. Go back as far as you can comfortably
remember. Do not include commitments of less than 25 hours/year.
- Portfolio of your creative work
- Most recent psycho-educational evaluation
to the First Boarding School Planning Meeting
Please bring with you as many of the following items as are
appropriate for you:
- Current report card and teacher progress reports
- Year-end report cards for past three years and corresponding
teacher progress reports
- Standardized testing results (SSAT, ISEE, CTBS, Stanford,
etc)
- PSAT, SAT, and ACT score reports (if applicable)
- Teacher evaluation forms (at least 2, if time permits)
- ON FLOPPY DISK: List of most important school-related
activities, outside activities, religious activities, hobbies
and interests, community service, summer experiences, and
honors. Oragnize the list by activity, from most important
to least important. Go back as far as you can comfortably
remember. Do not include commitments of less than 25 hours/year.

- Most recent psycho-educational evaluation (if applicable)
to the First Special Needs Meeting
Please bring with you as many of the following items as are
appropriate for you:
- Psycho-social history provided by parents preferably
on floppy disk or typed that will provide detailed
information about the student's behavioral and emotional
issues, using as many examples as possible. You may go back
as far as you think is necessary to make sure we have a
complete picture positives and negatives
from your perspective. Make sure to include birth date,
height, weight, relevant health issues, and current medications.
- Clinical summaries and diagnostic impressions (from therapists,
psychologists, psychiatrists)
- Most recent psycho-educational evaluation
- Current report card and teacher progress reports
- Year-end report cards for the past three years and teacher
progress reports
- Standardized testing results
- List of hobbies and interests
